Our provider gathers information that helps government agencies and private companies protect themselves and American consumers with industry-leading risk management products. They provide information only to those who can demonstrate a legitimate need for the information and who agree to abide by all local and federal laws and regulations regarding the use of the information. These laws include, but are not limited to, the Gramm-Leach-Bliley Act (Public Law 106-102, 15 USC §6801, et seq.), the Drivers Privacy Protection Act (18 USC §2721 et seq.) and the Fair Credit Reporting Act.
They also provide personally identifiable information, such as Social Security numbers (SSNs) and dates of birth (DOBs), only when a substantial business or government need for the data has been demonstrated by the end user. The end user must also demonstrate substantial security precautions to protect the personally identifiable information. In order to comply with these laws, Exhibit A () must be completed. Our provider may confirm these requirements through the use of a site inspection at cost to the customer. A site inspection order form is available by clicking here () .
You will also need to complete and submit the National FlatRate Usage Agreement.
